The Role of Anecdotes in Science-Based Medicine
While attending a lecture by a naturopath at my institution I had the opportunity to ask the following question: given the extreme scientific implausibility of homeopathy, and the overall negative clinical evidence, why do you continue to prescribe homeopathic remedies? The answer, as much as my question, exposed a core difference between scientific and sectarian health care providers. She said, “Because I have seen it work in my practice.”
There it is. She and many other practitioners of dubious modalities are compelled by anecdotal experience while I am not.
An anecdote is a story – in the context of medicine it often relates to an individual’s experience with their disease or symptoms and their efforts to treat it. People generally find anecdotes highly compelling, while scientists are deeply suspicious of anecdotes. We are fond of saying that the plural of anecdote is anecdotes, not data. Why is this?
Humans are social storytelling animals – we instinctively learn by the experience of others. My friend ate that plant with the bright red berries and then became very ill – lesson: don’t eat from that plant. This is a type of heuristic, a mental shortcut that humans evolved in order to make quick and mostly accurate judgments about their environment. From an evolutionary point of view it is probably statistically advantageous just to avoid the plant with the red berries rather than conduct blinded experiments to see if it really was the plant that made your friend sick.
Further, the most compelling stories are our own. When we believe we have experienced something directly, it is difficult to impossible to convince us otherwise. It’s just the way humans are hardwired.
Understanding the world through stories was a good strategy in the environment of our evolutionary history but is far too flawed to deal with the complex world we live in today. In fact, the discipline of science developed as a tool to go beyond the efficient but flawed techniques we evolved. Perhaps, for example, your friend became ill because of the raw eggs he consumed earlier in the day, and the plant had nothing to do with it. Evolutionary pressures favored a more simplistic approach to nature, one that tended to assume that apparent patterns were real.
In today’s modern society we are confronted with a dizzying array of apparent patterns and using the simple rules of thumb we evolved to deal with them is not adequate. Whether or not a treatment works for a symptom or disease is a good example. Symptoms tend to vary over time, some may spontaneously remit, and our perceptions of symptoms are susceptible to a host of psychological factors. There are also numerous biological factors that may have an effect. If we are to make reliable decisions about the effects of specific interventions on symptoms and diseases we will need to do better than uncontrolled observation, or anecdotes.
The primary weakness of anecdotes as evidence is that they are not controlled. This opens them up to many hidden variables that could potentially affect the results. We therefore cannot make any reliable assumptions about which variable (for example a specific treatment) was responsible for any apparent improvement.
Here are some specific factors that make it difficult to impossible to reliably interpret anecdotal medical evidence:
Regression to the mean: This is a statistical phenomenon whereby any extreme variation is likely to be followed by a more average variation – by chance alone. Many diseases have variable or fluctuating symptoms – good days and bad days, or periods of exacerbation followed by periods of relative relief. If a person seeks out a treatment when their symptoms are severe, by chance alone this is likely to be followed by a period when the symptoms are not as severe.
Most illnesses are self-limiting: The old saying goes that if you don’t treat a cold it will last for seven days, and if you treat it it will last for a week. Most ailments get better or improve on their own, therefore most treatments will be followed by symptom resolution even if the treatment has no biological effect. More broadly, all illnesses have a natural history, a course they typically follow over time. In order to know if a treatment is affecting that course it has to be compared to patients who are not treated, or receive a different treatment.
Multiple treatments: Often people will try multiple treatments for a disease or ailment making it impossible to tell which treatment had a beneficial effect, if any. Multiple treatments may be taken all at once, or sequentially. For example, a person with a long term illness (but one destined to have a period of relative relief) tries treatment A without effect, then treatment B without effect, then treatment C which is followed by improvements in their symptoms. They then credit treatment C, recounting how multiple other treatments had failed. However, since the person was trying some new treatment most of the time at any point that their symptoms improved there would be a treatment they could credit with that improvement.
Dead men tell no tales (the problem of reporting bias): Cancer survivor groups do not contain people who died of their cancer. Those who die of a disease are not around to give their anecdotes. There is therefore a built in reporting bias. Also, those who feel they were helped by a treatment are much more likely to boast about it than those for whom there was no apparent benefit. People like to tell the tale of the miracle cure they found and had faith in, despite the skeptics and naysayers – but their vision paid off as the treatment worked for them. People have no motivation to recount their experience with the novel treatment that did not work. Further, patients who feel they are being helped by their doctor or practitioner are more likely to return. Those who feel the treatments are not working may not come back at all to report the treatment failure.
Confirmation bias: It is a well-described psychological phenomenon that we tend to seek out and remember information that confirmed what we already believe, or want to believe, and we avoid, forget, or explain away disconfirming evidence.
Vague outcome measures: Good clinical trials use objective outcome measures – those that are binary (like death or survival), quantitative (like a blood level), or are based upon a specific physical finding. Subjective symptoms do not make good outcome measures because they require that judgments be made, and that introduces yet another variable. Should you count those mild sniffles as having a cold? If you are taking a remedy that you think will help you avoid colds you may dismiss those sniffles and report (and even remember) that you did not get any colds while taking the treatment.
The Placebo Effect: The placebo effect is actually a host of many effects that give the appearance of a response to an inactive treatment. These factors include many of the things I listed above, but also other variables that may alter health outcomes or symptoms. See here for a more complete discussion.
The Fallibility of Human Memory: Medical students quickly learn that one of the biggest challenges in taking a medical history is that people are poor historians, which a polite way of saying that human memory is terrible. Anecdotes largely depend upon an individual’s memory of their illness and treatment. This introduces many new variables. There is, for example, a tendency for people to conflate different events in their memory into a single event, or to combine details from various events. There is also a tendency for details to evolve over time to make a story more clean and profound. So people may, in their memory, exaggerate the severity of their symptoms prior to treatment, exaggerate the response to the treatment, clean up the timeline of events so that improvement began very soon after a treatment (rather than before or long after), forget other treatments that were taken, distort what they were told by their various health care providers, etc. I have had countless opportunities to compare a patient’s memory of their illness and treatment to the documented medical records, and the correlation ranges from poor to completely wrong.
For these, and other reasons, scientists have learned not to trust anecdotal reports – or rather to have a realistic assessment of their reliability. This is why it always strikes me as profoundly naive when anyone presents anecdotal evidence as if it is compelling, or even argues that anecdotes should be relied upon as valid evidence.
We also have history to inform our opinions about anecdotes. Western practitioners relied upon the humoral theory of health and illness for thousands of years. Apparently thousands of years of anecdotal experience did not inform them that their treatments were worthless or harmful. Dr. Abrams became wealthy by selling a machine to diagnosis and treat ailments. His devices were widely used, with millions of people swearing by their effectiveness. It worked for them, and their experience was unshakable. When Abrams died it was discovered that his machines (previously protected from inspection) were filled with useless random machine parts. At the turn of the century radioactive tonics were popular, until prominent proponents began seeing the ill effects of radiation poisoning.
The point of these examples is that anecdotal evidence led many people to conclude that these interventions worked. They are useful examples because they are no longer accepted, humoral theory was replaced by scientific medicine, Abrams devices were dramatically exposed, and radiation therapy is directly harmful. But for treatments that are not directly harmful (and least not in an obvious way) or where there is no “man behind the curtain” to dramatically expose, all we have are the anecdotes – and clearly they are not reliable.
Even in mainstream medicine we have learned to distrust anecdotal evidence, even our own. The history of medicine is strewn with treatments that seemed to work but then were abandoned when scientific evidence showed otherwise. The classic example of this is mammary artery bypass for cardiac angina – it seemed anecdotally to work, but it didn’t.
But should anecdotes play any role in medical evidence? Yes, but a very minor and clearly defined one. Anecdotes, with all their weaknesses, are real life experience. It is possible that a treatment does in fact work and personal experience may be the first indication that there is a meaningful biological effect in play. But here are two limiting factors in how anecdotes should be incorporated into medical evidence:
The first is that anecdotes should be documented as carefully as possible. This is a common practice in scientific medicine, where anecdotes are called case reports (when reported individually) or a case series (when a few related anecdotes are reported). Case reports are anecdotal because they are retrospective and not controlled. But it can be helpful to relay a case where all the relevant information is carefully documented – the timeline of events, all treatments that were given, test results, exam findings, etc. This at least locks this information into place and prevents further distortion by memory. It also attempts to document as many confounding variables as possible.
The second criterion for the proper use of anecdotes in scientific medicine is that they should be thought of as preliminary only – as a means of pointing the way to future research. They should never be considered as definitive or compelling by themselves. Any findings or conclusions suggested by anecdotal case reports need to be later verified by controlled prospective clinical studies.
Understanding the nature and role of anecdotes is vital to bridging the gap between the proponents of science-based medicine and believers in dubious or sectarian health practices (as well as the public at large). In my experience it is often the final point of contention between these two camps.
It is interesting to note that the scientific community has long ago made up its collective mind about the weaknesses and role of anecdotes. Logic and the lessons of history speak very clearly on this issue. But there are forces at work today that want to turn back the clock on scientific progress – they want to bring back anecdotes as a reliable source of medical evidence, essentially returning to the pre-scientific era of medicine. In some cases this is done out of frustration – that controlled scientific data has not validated a prior strongly held belief. In other cases it seems to be a calculated attempt to lower the bar of evidence to admit treatments that have not been validated by solid scientific evidence. In either case, this is not in the best interest of the health of the public.

- Sensationalized media: Often, the media presents outlier case studies due to their sensational nature. But media reports are often simply sensational anecdotal evidence that the reporters have found rather than statistical data.
- Bad parenting: A man decides his neighbor is a terrible parent because he saw her one day unable to control her child. He didn’t see all the other examples of when that woman was being an excellent mother
- Observing that Opposites Attract: Sometimes,anecdotal evidence can be accurate. For example, the common saying that “opposites attract” is based on anecdotal evidence of people observing the oddity of couples that don’t seem to match. Lately, research has studied the validity of this anecdotal evidence. The key finding: romantic relationships are about balance .
- Life Lessons: Life lessons passed on from generation to generation may not be based in the scientific method , the this kind of anecdotal evidence has a lot of credibility nonetheless. For example, a father who tells his son not to get involved in a bad relationship comes from his anecdotal experience, but it’s also great advice.
- Home Remedies: People often share personal anecdotes about the effectiveness of home remedies for various ailments. These stories are based on individual experiences rather than scientific evidence, but they can still influence others to try the remedies themselves.
- Career Advice: A successful professional may offer career advice based on their personal experiences, attributing their success to certain behaviors or decisions. While these anecdotes might be inspiring, they don’t necessarily guarantee similar results for everyone who follows the same path.
- Anecdotal Evidence in Scientific Communication: When anecdotal evidence comes from a trusted expert, it can be very persuasive . As a result, scientists often teach concepts like climate change by appealing to anecdotal evidence (e.g. ‘you’ve noticed it’s gotten hotter since your childhood’). This is actually part of Petty and Cacioppo’s (1986) Elaboration Likelihood Model of Persuasion .
- Product Reviews: A friend raves about a new gadget or appliance that they recently purchased, claiming it has made their life easier. This anecdotal evidence might be compelling, but it doesn’t provide a comprehensive evaluation of the product’s overall quality and effectiveness.
- Superstitions and Beliefs: People may believe in certain superstitions or rituals based on anecdotal evidence. For example, someone might avoid walking under ladders due to a story they heard about someone experiencing bad luck after doing so, despite the lack of any statistical correlation between the action and the outcome.
- Stereotyping: We’ve all heard the saying “birds of a feather flock together.” This is an example of anecdotal evidence where a person stereotypes a whole group of people based upon their experience with one person from within that group.
- Weight Loss Success: Individuals might share stories of losing weight through a specific diet or exercise routine. While these anecdotes can be motivating, they may not be universally applicable or supported by scientific research on the most effective weight loss methods.
- Pet Behavior: Pet owners may share anecdotes about their pets’ unique behaviors or training techniques that have worked for them. These stories, while interesting, might not be applicable to all pets or provide reliable information about animal behavior in general.
- Tennis Player’s Routine: A tennis player has the best game of his life one day after eating three eggs for breakfast. He’s so convinced by this anecdotal evidence that every game day has two start with three eggs and a big cup of coffee or else he’s convinced he will lose the game.
- Effective Study Habits : Students might claim that a specific study technique such as the pomodoro method led to their academic success. So they tell their friends to follow the advice, too. Unfortunately, the pomodoro technique may not work for everyone as we all have our own unique motivations and learning preferences.
- Lucky Charms: A person might carry around a lucky charm in their wallet because the day they found the lucky charm was a very good day for them. They suggest others buy these lucky charms, too, based on their own anecdotal evidence about the charms.
Levels of Evidence: Anecdotal Evidence Ranks Low in Reliability
Practicing psychologists (APA, 2006) rely on a hierarchy of evidence which identifies the degree of validity of evidence based on stringent research parameters.
According to Stegenga (2014), “An evidence hierarchy is a rank-ordering of kinds of methods according to the potential for that method to suffer from systematic bias” (p. 313).

As seen in this graphic, at the bottom of the hierarchy is anecdotal evidence, which carries no scientific weight and has the highest degree for potential bias.
Is Anecdotal Evidence Ever Useful?
Although anecdotal evidence is not grounded in science or produced as a result of a scientific study, it does have some limited value.
For instance, a person’s description about an experience can be used as a starting point to understanding a given phenomenon.
If several people have similar descriptions, then it may inform a scientist as to common characteristics of an experience which may be the impetus for a scientific inquiry.
In other cases, it is the errors in judgement as a result of anecdotal recollections that can lead to insights into perception and decision-making.
Case Studies of Anecdotal Evidence
1. the many insights of jean piaget.
Jean Piaget was a Swiss psychologist that developed an incredibly insightful theory of cognitive development. It is a theory that has driven decades of research and become a cornerstone of developmental psychology.
Even though Piaget was a trained professional, his methodology was nearly 100% anecdotal. He observed each of his children and took exhaustive notes on their behavior.
On the emergence of the deliberate action of a newborn, as evidence of intent:
“The child discovers in this way that which has been called in scientific language the “experiment in order to see” (p. 266). The infant is attempting the “discovery of new means through active experimentation” (Piaget, 1956, p. 267).A few years later, Piaget provides anecdotal evidence for what will become officially known as egocentrism . Here is a sample of his observations from The Language and Thought of the Child (1959).
“Our notes show…that at the beginning of his fourth year the child’s speech shows a greater coefficient of ego-centrism (i.e., it is less socialized in character) when speaking with adults than children of his own age (71.2% against 56.2%)” (p. 143).
Piaget had no right to make scientific claims based on this data – it’s purely anecdotal. Nevertheless, it still rings true to this day. Sometimes, anecdotal evidence turns out to be true.
2. Anecdotal Evidence and Criminal Prosecutions
Every year in the United States, thousands of people are convicted of crimes based on eyewitness testimony. That is, based on anecdotal evidence. Most of those witnesses are quite confident in their accounts.
However, psychologist Dr. Elizabeth Loftus (1997) has demonstrated that this form of anecdotal evidence can be quite unreliable.
In fact, under certain conditions, false memories can be created that “feel” accurate, but are not.
“False memories are constructed by combining actual memories with the content of suggestions received from others” ( Loftus, 1997 , p. 75).
One of the first studies was conducted by Loftus and Palmer (1974), which demonstrated that memory could be easily altered simply by changing the way a question is phrased.
This line of research has had a tremendous impact on law enforcement and the credibility of eyewitness testimony .
Anecdotal evidence refers to information that comes from our own experience or someone else’s. It can be very persuasive, especially if it comes from a credible source, such as an expert or trusted friend.
As a source of scientific evidence however, it is considered unreliable. There are many possible biases that can undermine the validity of anecdotal evidence.
Although often harmless, in some contexts, anecdotal evidence can be quite dangerous. For example, research has demonstrated that eyewitness testimony can be easily manipulated.
On the other side of the coin however, anecdotal evidence can be the impetus for a new line of scientific inquiry.
Nearly all of Piaget’s theory of cognitive development was based on anecdotal evidence he gathered by observing his own children. And let’s not forget that our understanding of bystander intervention was sparked by anecdotal accounts of a terrible tragedy.
So, while scientists tend to discount the value of anecdotal evidence, it has played a significant role in many highly influential in psychological studies.

Evidence-based practice for effective decision-making
Effective HR decision-making is based on considering the best available evidence combined with critical thinking.
People professionals are faced with complex workplace decisions and need to understand ‘what works’ in order to influence organisational outcomes for the better.
Evidence-based practice helps them make better, more effective decisions by choosing reliable, trustworthy solutions and being less reliant on outdated received wisdom, fads or superficial quick fixes.
At the CIPD, we believe this is an important step for the people profession to take: our Profession Map describes a vision of a profession that is principles-led, evidence-based and outcomes-driven. Taking an evidence-based approach to decision-making can have a huge impact on the working lives of people in all sorts of organisations worldwide.
This factsheet outlines what evidence-based practice is and why it is so important, highlighting the four sources of evidence to draw on and combine to ensure the greatest chance of making effective decisions. It then looks to the steps we can take to move towards an evidence-based people profession.

At the heart of evidence-based practice is the idea that good decision-making is achieved through critical appraisal of the best available evidence from multiple sources. When we say ‘evidence’, we mean information, facts or data supporting (or contradicting) a claim, assumption or hypothesis. This evidence may come from scientific research, the local organisation, experienced professionals or relevant stakeholders. We use the following definition from CEBMa :
“Evidence-based practice is about making decisions through the conscientious, explicit and judicious use of the best available evidence from multiple sources… to increase the likelihood of a favourable outcome.”
In search of best available evidence
The reasons why evidence-based practice is so important, the principles that underpin it, how it can be followed and how challenges in doing so can be overcome.
Information overload
In their report Evidence-based management: the basic principles , Eric Barends, Denise Rousseau and Rob Briner of CEBMa outline the challenge of biased and unreliable management decisions.
People professionals face all sorts of contradictory insights and claims about what works and what doesn’t in the workplace. As Daniel Levitin puts it:
"We're assaulted with facts, pseudo facts, jibber-jabber, and rumor, all posing as information. Trying to figure out what you need to know and what you can ignore is exhausting."
While assessing the reliability of evidence becomes more important as the mass of opinion grows, with such a barrage of information, we inevitably use mental shortcuts to make decisions easier and to avoid our brains overloading.
Unfortunately, this means we are prone to biases. Our reports a head for hiring and our minds at work outline the most common of these:
- Authority bias: the tendency to overvalue the opinion of a person or organisation that is seen as an authority
- Conformity bias: the tendency to conform to others in a group, also referred to as 'group think' or 'herd behaviour'
- Confirmation bias: looking to confirm existing beliefs when assessing new information
- Patternicity or the illusion of causality: the tendency to see patterns and assume causal relations by connecting the dots even when there is just random 'noise'.
So-called ‘best practice’
Received wisdom and the notion of ‘best practice’ also creates bias. One organisation may look to another as an example of sound practice and decision-making, without critically evaluating the effectiveness of their actions. And while scientific literature on key issues in the field is vital, there’s a gap between this and the perceptions of practitioners, who are often unaware of the depth of research available.
Cherry-picking evidence
Even when looking at research, we can be naturally biased. We have a tendency to ‘cherry-pick’ research that backs up a perspective or opinion and ignores research that does not, even if it gives stronger evidence on cause-and-effect relationships. This bad habit is hard to avoid – it's even common among academic researchers. So we need approaches that help us determine which research evidence we should trust.

How can evidence-based practice help?
Our thought leadership article outlines the importance of evidence-based practice in more detail but, essentially, it has three main benefits:
- It ensures that decision-making is based on fact, rather than outdated insights, short-term fads and natural bias.
- It creates a stronger body of knowledge and as a result, a more trusted profession.
- It gives more gravitas to professionals, leads to increased influence on other business leaders and has a more positive impact in work.
The four sources of evidence
The issues above demonstrate the limitations of basing decisions on limited, unreliable evidence. Before making an important decision or introducing a new practice, an evidence-based people professional should start by asking: "What is the available evidence?" As a minimum, people professionals should consider four sources of evidence.
- Scientific literature on people management has become more readily available in recent years, particularly on topics such as the recruitment and selection of personnel, the effect of feedback on performance and the characteristics of effective teams. People professionals’ ability to search for and appraise research for its relevance and trustworthiness is essential.
- Organisational data must be examined as it highlights issues needing a manager’s attention. This data can come externally from customers or clients (customer satisfaction, repeated business), or internally from employees (levels of job satisfaction, retention rates). There’s also the comparison between ‘hard’ evidence, such as turnover rate and productivity levels, and ‘soft’ elements, like perceptions of culture and attitudes towards leadership. Gaining access to organisational data is key to determining causes of problems, and finding and implementing solutions.
- Expertise and judgement of practitioners, managers, consultants and business leaders is important to ensure effective decision-making. This professional knowledge differs from opinion as it’s accumulated over time through reflection on outcomes of similar actions taken in similar contexts. It reflects specialised knowledge acquired through repeated experience of specialised activities.
- Stakeholders, both internal (employees, managers, board members) and external (suppliers, investors, shareholders), may be affected by an organisation’s decisions and their consequences. Their values reflect what they deem important, which in turn affects how they respond to the organisation’s decisions. Acquiring knowledge of their concerns provides a frame of reference for analysing evidence.
Combining the evidence
One very important element of evidence-based practice is collating evidence from different sources. There are six ways – depicted in our infographic below – which will encourage this:

- Asking – translating a practical issue or problem into an answerable question.
- Acquiring – systematically searching for and retrieving evidence.
- Appraising – critically judging the trustworthiness and relevance of the evidence.
- Aggregating – weighing and pulling together the evidence.
- Applying – incorporating the evidence into a decision-making process.
- Assessing – evaluating the outcome of the decision taken so as to increase the likelihood.
Through these six steps, practitioners can ensure the quality of evidence is not ignored. Appraisal varies depending on the source of evidence, but generally involves the same questions:
- Where and how is evidence gathered?
- Is it the best evidence available?
- Is it sufficient to reach a conclusion?
- Might it be biased in a particular direction? If so, why?
Evidence-based practice is about using the best available evidence from multiple sources to optimise decisions. Being evidence-based is not a question of looking for ‘proof’, as this is far too elusive. However, we can – and should – prioritise the most trustworthy evidence available. The gains in making better decisions on the ground, strengthening the body of knowledge and becoming a more influential profession are surely worthwhile.
To realise the vision of a people profession that’s genuinely evidence-based, we need to move forward on two fronts.
First, we need to make sure that the body of professional knowledge is evidence-based – the CIPD’s Evidence review hub is one way in which we are doing this.
Second, people professionals need to develop capacity in engaging with the best available evidence. Doing this as a non-researcher may feel daunting, but taking small steps towards more evidence-based decisions can make a huge difference. Our thought leadership article outlines a maturity model for being more evidence-based in more detail, but to summarise, we’d encourage people professionals to take the following steps:
- Read research : engage with high-quality research on areas of interest through reading core textbooks and journals that summarise research.
- Collect and analyse organisational data : in the long-term, developing analytical capability should be an aim for the people profession. More immediately, HR leaders should have some knowledge of data-analytics, enough to ask probing questions and make the case for the resources needed for robust measures.
- Review published evidence , including conducting or commissioning short evidence reviews of scientific literature to inform decisions.
- Pilot new practices : evaluate new interventions through applying the same principles used in rigorous cause-and-effect research.
- Share your knowledge : strengthen the body of knowledge by sharing research insights at events or in publications.
- Critical thinking : throughout this process, question assumptions and carefully consider where there are gaps in knowledge.
Developing this sort of capability is a long journey but one that people professionals should aspire to. As the professional body for HR and people development, the CIPD takes an evidence-based view on the future of work – and, importantly, what this means for our profession. By doing this, we can help prepare professionals and employers for what’s coming, while also equipping them to succeed and shape a changing world of work.
Our Profession Map has been developed to do this. It defines the knowledge, behaviours and values which should underpin today’s people profession. It has been developed as an international standard against which an organisation can benchmark its values. At its core are the concepts of being principles-led, evidence-based and outcomes driven. This recognises the importance of using the four forms of evidence in a principled manner to develop positive outcomes for stakeholders. As evidence is often of varying degrees of quality, it’s important that people professionals consider if and how they should incorporate the different types of evidence into their work.
Evidence-based practice is a useful concept for understanding whether practices in HR lead to the desired outcomes, and whether these practices are being used to the best effect.

Distinguishing case study as a research method from case reports as a publication type
The purpose of this editorial is to distinguish between case reports and case studies. In health, case reports are familiar ways of sharing events or efforts of intervening with single patients with previously unreported features. As a qualitative methodology, case study research encompasses a great deal more complexity than a typical case report and often incorporates multiple streams of data combined in creative ways. The depth and richness of case study description helps readers understand the case and whether findings might be applicable beyond that setting.
Single-institution descriptive reports of library activities are often labeled by their authors as “case studies.” By contrast, in health care, single patient retrospective descriptions are published as “case reports.” Both case reports and case studies are valuable to readers and provide a publication opportunity for authors. A previous editorial by Akers and Amos about improving case studies addresses issues that are more common to case reports; for example, not having a review of the literature or being anecdotal, not generalizable, and prone to various types of bias such as positive outcome bias [ 1 ]. However, case study research as a qualitative methodology is pursued for different purposes than generalizability. The authors’ purpose in this editorial is to clearly distinguish between case reports and case studies. We believe that this will assist authors in describing and designating the methodological approach of their publications and help readers appreciate the rigor of well-executed case study research.
Case reports often provide a first exploration of a phenomenon or an opportunity for a first publication by a trainee in the health professions. In health care, case reports are familiar ways of sharing events or efforts of intervening with single patients with previously unreported features. Another type of study categorized as a case report is an “N of 1” study or single-subject clinical trial, which considers an individual patient as the sole unit of observation in a study investigating the efficacy or side effect profiles of different interventions. Entire journals have evolved to publish case reports, which often rely on template structures with limited contextualization or discussion of previous cases. Examples that are indexed in MEDLINE include the American Journal of Case Reports , BMJ Case Reports, Journal of Medical Case Reports, and Journal of Radiology Case Reports . Similar publications appear in veterinary medicine and are indexed in CAB Abstracts, such as Case Reports in Veterinary Medicine and Veterinary Record Case Reports .
As a qualitative methodology, however, case study research encompasses a great deal more complexity than a typical case report and often incorporates multiple streams of data combined in creative ways. Distinctions include the investigator’s definitions and delimitations of the case being studied, the clarity of the role of the investigator, the rigor of gathering and combining evidence about the case, and the contextualization of the findings. Delimitation is a term from qualitative research about setting boundaries to scope the research in a useful way rather than describing the narrow scope as a limitation, as often appears in a discussion section. The depth and richness of description helps readers understand the situation and whether findings from the case are applicable to their settings.
CASE STUDY AS A RESEARCH METHODOLOGY
Case study as a qualitative methodology is an exploration of a time- and space-bound phenomenon. As qualitative research, case studies require much more from their authors who are acting as instruments within the inquiry process. In the case study methodology, a variety of methodological approaches may be employed to explain the complexity of the problem being studied [ 2 , 3 ].
Leading authors diverge in their definitions of case study, but a qualitative research text introduces case study as follows:
Case study research is defined as a qualitative approach in which the investigator explores a real-life, contemporary bounded system (a case) or multiple bound systems (cases) over time, through detailed, in-depth data collection involving multiple sources of information, and reports a case description and case themes. The unit of analysis in the case study might be multiple cases (a multisite study) or a single case (a within-site case study). [ 4 ]
Methodologists writing core texts on case study research include Yin [ 5 ], Stake [ 6 ], and Merriam [ 7 ]. The approaches of these three methodologists have been compared by Yazan, who focused on six areas of methodology: epistemology (beliefs about ways of knowing), definition of cases, design of case studies, and gathering, analysis, and validation of data [ 8 ]. For Yin, case study is a method of empirical inquiry appropriate to determining the “how and why” of phenomena and contributes to understanding phenomena in a holistic and real-life context [ 5 ]. Stake defines a case study as a “well-bounded, specific, complex, and functioning thing” [ 6 ], while Merriam views “the case as a thing, a single entity, a unit around which there are boundaries” [ 7 ].
Case studies are ways to explain, describe, or explore phenomena. Comments from a quantitative perspective about case studies lacking rigor and generalizability fail to consider the purpose of the case study and how what is learned from a case study is put into practice. Rigor in case studies comes from the research design and its components, which Yin outlines as (a) the study’s questions, (b) the study’s propositions, (c) the unit of analysis, (d) the logic linking the data to propositions, and (e) the criteria for interpreting the findings [ 5 ]. Case studies should also provide multiple sources of data, a case study database, and a clear chain of evidence among the questions asked, the data collected, and the conclusions drawn [ 5 ].
Sources of evidence for case studies include interviews, documentation, archival records, direct observations, participant-observation, and physical artifacts. One of the most important sources for data in qualitative case study research is the interview [ 2 , 3 ]. In addition to interviews, documents and archival records can be gathered to corroborate and enhance the findings of the study. To understand the phenomenon or the conditions that created it, direct observations can serve as another source of evidence and can be conducted throughout the study. These can include the use of formal and informal protocols as a participant inside the case or an external or passive observer outside of the case [ 5 ]. Lastly, physical artifacts can be observed and collected as a form of evidence. With these multiple potential sources of evidence, the study methodology includes gathering data, sense-making, and triangulating multiple streams of data. Figure 1 shows an example in which data used for the case started with a pilot study to provide additional context to guide more in-depth data collection and analysis with participants.

VARIATIONS ON CASE STUDY METHODOLOGY
Case study methodology is evolving and regularly reinterpreted. Comparative or multiple case studies are used as a tool for synthesizing information across time and space to research the impact of policy and practice in various fields of social research [ 9 ]. Because case study research is in-depth and intensive, there have been efforts to simplify the method or select useful components of cases for focused analysis. Micro-case study is a term that is occasionally used to describe research on micro-level cases [ 10 ]. These are cases that occur in a brief time frame, occur in a confined setting, and are simple and straightforward in nature. A micro-level case describes a clear problem of interest. Reporting is very brief and about specific points. The lack of complexity in the case description makes obvious the “lesson” that is inherent in the case; although no definitive “solution” is necessarily forthcoming, making the case useful for discussion. A micro-case write-up can be distinguished from a case report by its focus on briefly reporting specific features of a case or cases to analyze or learn from those features.
DATABASE INDEXING OF CASE REPORTS AND CASE STUDIES
Disciplines such as education, psychology, sociology, political science, and social work regularly publish rich case studies that are relevant to particular areas of health librarianship. Case reports and case studies have been defined as publication types or subject terms by several databases that are relevant to librarian authors: MEDLINE, PsycINFO, CINAHL, and ERIC. Library, Information Science & Technology Abstracts (LISTA) does not have a subject term or publication type related to cases, despite many being included in the database. Whereas “Case Reports” are the main term used by MEDLINE’s Medical Subject Headings (MeSH) and PsycINFO’s thesaurus, CINAHL and ERIC use “Case Studies.”
Case reports in MEDLINE and PsycINFO focus on clinical case documentation. In MeSH, “Case Reports” as a publication type is specific to “clinical presentations that may be followed by evaluative studies that eventually lead to a diagnosis” [ 11 ]. “Case Histories,” “Case Studies,” and “Case Study” are all entry terms mapping to “Case Reports”; however, guidance to indexers suggests that “Case Reports” should not be applied to institutional case reports and refers to the heading “Organizational Case Studies,” which is defined as “descriptions and evaluations of specific health care organizations” [ 12 ].
PsycINFO’s subject term “Case Report” is “used in records discussing issues involved in the process of conducting exploratory studies of single or multiple clinical cases.” The Methodology index offers clinical and non-clinical entries. “Clinical Case Study” is defined as “case reports that include disorder, diagnosis, and clinical treatment for individuals with mental or medical illnesses,” whereas “Non-clinical Case Study” is a “document consisting of non-clinical or organizational case examples of the concepts being researched or studied. The setting is always non-clinical and does not include treatment-related environments” [ 13 ].
Both CINAHL and ERIC acknowledge the depth of analysis in case study methodology. The CINAHL scope note for the thesaurus term “Case Studies” distinguishes between the document and the methodology, though both use the same term: “a review of a particular condition, disease, or administrative problem. Also, a research method that involves an in-depth analysis of an individual, group, institution, or other social unit. For material that contains a case study, search for document type: case study.” The ERIC scope note for the thesaurus term “Case Studies” is simple: “detailed analyses, usually focusing on a particular problem of an individual, group, or organization” [ 14 ].
PUBLICATION OF CASE STUDY RESEARCH IN LIBRARIANSHIP
We call your attention to a few examples published as case studies in health sciences librarianship to consider how their characteristics fit with the preceding definitions of case reports or case study research. All present some characteristics of case study research, but their treatment of the research questions, richness of description, and analytic strategies vary in depth and, therefore, diverge at some level from the qualitative case study research approach. This divergence, particularly in richness of description and analysis, may have been constrained by the publication requirements.
As one example, a case study by Janke and Rush documented a time- and context-bound collaboration involving a librarian and a nursing faculty member [ 15 ]. Three objectives were stated: (1) describing their experience of working together on an interprofessional research team, (2) evaluating the value of the librarian role from librarian and faculty member perspectives, and (3) relating findings to existing literature. Elements that signal the qualitative nature of this case study are that the authors were the research participants and their use of the term “evaluation” is reflection on their experience. This reads like a case study that could have been enriched by including other types of data gathered from others engaging with this team to broaden the understanding of the collaboration.
As another example, the description of the academic context is one of the most salient components of the case study written by Clairoux et al., which had the objectives of (1) describing the library instruction offered and learning assessments used at a single health sciences library and (2) discussing the positive outcomes of instruction in that setting [ 16 ]. The authors focus on sharing what the institution has done more than explaining why this institution is an exemplar to explore a focused question or understand the phenomenon of library instruction. However, like a case study, the analysis brings together several streams of data including course attendance, online material page views, and some discussion of results from surveys. This paper reads somewhat in between an institutional case report and a case study.
The final example is a single author reporting on a personal experience of creating and executing the role of research informationist for a National Institutes of Health (NIH)–funded research team [ 17 ]. There is a thoughtful review of the informationist literature and detailed descriptions of the institutional context and the process of gaining access to and participating in the new role. However, the motivating question in the abstract does not seem to be fully addressed through analysis from either the reflective perspective of the author as the research participant or consideration of other streams of data from those involved in the informationist experience. The publication reads more like a case report about this informationist’s experience than a case study that explores the research informationist experience through the selection of this case.
All of these publications are well written and useful for their intended audiences, but in general, they are much shorter and much less rich in depth than case studies published in social sciences research. It may be that the authors have been constrained by word counts or page limits. For example, the submission category for Case Studies in the Journal of the Medical Library Association (JMLA) limited them to 3,000 words and defined them as “articles describing the process of developing, implementing, and evaluating a new service, program, or initiative, typically in a single institution or through a single collaborative effort” [ 18 ]. This definition’s focus on novelty and description sounds much more like the definition of case report than the in-depth, detailed investigation of a time- and space-bound problem that is often examined through case study research.
Problem-focused or question-driven case study research would benefit from the space provided for Original Investigations that employ any type of quantitative or qualitative method of analysis. One of the best examples in the JMLA of an in-depth multiple case study that was authored by a librarian who published the findings from her doctoral dissertation represented all the elements of a case study. In eight pages, she provided a theoretical basis for the research question, a pilot study, and a multiple case design, including integrated data from interviews and focus groups [ 19 ].
We have distinguished between case reports and case studies primarily to assist librarians who are new to research and critical appraisal of case study methodology to recognize the features that authors use to describe and designate the methodological approaches of their publications. For researchers who are new to case research methodology and are interested in learning more, Hancock and Algozzine provide a guide [ 20 ].
We hope that JMLA readers appreciate the rigor of well-executed case study research. We believe that distinguishing between descriptive case reports and analytic case studies in the journal’s submission categories will allow the depth of case study methodology to increase. We also hope that authors feel encouraged to pursue submitting relevant case studies or case reports for future publication.

Sources are considered primary, secondary, or tertiary depending on the originality of the information presented and their proximity or how close they are to the source of information. This distinction can differ between subjects and disciplines.
In the sciences, research findings may be communicated informally between researchers through email, presented at conferences (primary source), and then, possibly, published as a journal article or technical report (primary source). Once published, the information may be commented on by other researchers (secondary sources), and/or professionally indexed in a database (secondary sources). Later the information may be summarized into an encyclopedic or reference book format (tertiary sources). Source

A primary source in science is a document or record that reports on a study, experiment, trial or research project. Primary sources are usually written by the person(s) who did the research, conducted the study, or ran the experiment, and include hypothesis, methodology, and results.

Systematic reviews – Systematic reviews are best for answering single questions (eg, the effectiveness of tight glucose control on microvascular complications of diabetes). They are more scientifically structured than traditional reviews, being explicit about how the authors attempted to find all relevant articles, judge the scientific quality of each study, and weigh evidence from multiple studies with conflicting results. These reviews pay particular attention to including all strong research, whether or not it has been published, to avoid publication bias (positive studies are preferentially published). Source
Meta-analysis -- Meta-analysis, which is commonly included in systematic reviews, is a statistical method that quantitatively combines the results from different studies. It can be used to provide an overall estimate of the net benefit or harm of an intervention, even when these effects may not have been apparent in the individual studies [ 9 ].
